Providing Individualized Diabetes Management

A Certified Diabetes Care and Education Specialist (CDCES) is a health professional who possesses comprehensive knowledge of and experience in diabetes prevention, prediabetes, and diabetes management. A CDCES educates, supports, and advocates for people affected by diabetes, addressing the stages of diabetes throughout the lifespan. A CDCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.

If you have diabetes, you know how challenging it can be to manage your disease. Diabetes can impact a person at home, work, and school and effects many different areas of their life—diet, lifestyle, and overall health. Successful diabetes self-management involves knowledge about a broad range of topics such as nutrition, medication, monitoring systems, exercise and coping skills. It’s not easy to gain all of the necessary knowledge without help. This is where Halle Elbling, your CDE comes in. She can work with you to develop a plan to stay healthy, from healthy eating recommendations to understanding how to reduce diabetes risks and she can give you the tools and ongoing support to make that plan a regular part of your life. You can thrive with diabetes, and Halle can help.
