The Tandem t:flex Insulin Cartridge is designed for use with the Tandem t:flex Insulin Pump and is great for those who need a higher insulin capacity. Each cartridge holds up to 4.8mL (480 units) of insulin, meaning fewer cartridge changes and more flexibility in managing your diabetes.
These Tandem insulin cartridges feature t:lock luer-lock connectors, which make the filling process faster and more efficient. Compared to standard connectors, t:lock can reduce fill time by around 30 seconds and lower insulin waste by 4.4 units during each fill. For best results, always use the syringe and needle provided by Tandem Diabetes Care when filling the cartridge. After filling, and before connecting the tubing, hold your Tandem t:flex pump with the insulin fill port facing up to help reduce the chance of trapped air.
Tandem t:flex insulin cartridges are single-use only and should be replaced every 48 hours if you are using Humalog®, or every 72 hours if using Novolog®. They are only compatible with infusion sets that use the t:lock connection system and cannot be used with other types of connectors.

How to Fill the Tandem t:flex Insulin Cartridge
- Check the cartridge package for damage. If it looks damaged, throw it away and use a new one.
- Open the package and take out the cartridge.
- Hold the cartridge upright. Gently insert the needle into the white fill port. The needle won’t go all the way in—don’t push too hard.
- Keep the syringe straight up and down, with the needle still in the fill port. Pull the plunger all the way back to remove any air from the cartridge. You’ll see bubbles rise.
- While keeping the needle in place, let go of the plunger. It will move back to the neutral position on its own. This won’t push air back into the cartridge.
- Take the needle out of the fill port.
- Turn the syringe upright and gently pull the plunger down. Tap the syringe to help any bubbles rise to the top.
- Slowly push the plunger until a drop of insulin appears at the needle tip. This removes air from the syringe.
- Put the needle back into the fill port and slowly press the plunger to fill the cartridge with insulin. You may feel a little resistance—this is normal.
- Keep light pressure on the plunger while removing the needle. Check for leaks. If insulin is leaking, throw the cartridge away and start again with a new one.
- Always dispose of used needles, syringes, cartridges, and infusion sets safely, following local rules.
How to Install a Tandem t:flex Insulin Cartridge
Note: Do NOT remove the used cartridge from the pump during the load process until prompted on the t:flex Pump screen.
- From the Home Screen on your pump, tap OPTIONS.
- Tap LOAD.
- Tap CHANGE CARTRIDGE.
- The screen will let you know that insulin delivery will stop. Tap YES to move forward.
- Disconnect your infusion set from your body, then tap NEXT.
- Take out the used cartridge and insert the new, filled one. When you’re done, tap the UNLOCK icon.
- Tap NEXT to finish.
